The memento of appreciation will be presented at the annual parish assembly on Friday, May 18 and will be held by the recipient for a year.
Nomination forms can be obtained from Parish Clerk, Tony Allen at the parish office in Churchfields, Nutley.
They must completed and submitted by April 6 this year.
