Majorie Mosely, 84, of Two Mile Ash Road, Barns Green, was admitted to Iris Ward in Horsham when her mental health deteriorated.

However, she fell over twice in the hospital, fracturing her hip and was taken to East Surrey Hospital – where she died following two operations.

An inquest into her death was held at County Hall North, in Horsham on August 11.

Wendy Wallace, Mrs Moseley’s daughter said her mother ‘refused most of her help’.

In a statement read by Chris Wilkinson, deputy coroner for West Sussex, Mrs Wallace stated: “My mother was always extremely fit and active.

“By the time my mum was 80 she was taking little care of herself . Mum finally agreed to accept some support from social services. The situation was so awful we were referred to the dementia crisis team.”

The court heard Mrs Moseley had not been to see her GP for nearly ten years and was not formally diagnosed with dementia.

Sheila Cullen, deputy ward manager at Iris Ward, which is run by Sussex Partnership NHS Foundation Trust, said Mrs Moseley was beginning to ‘settle in’ and was making friends.

But she fell over twice despite being deemed ‘low risk’ and was taken to hospital for a hip operation.

Dr Iain Wilkinson, consultant at Surrey and Sussex Healthcare NHS Trust, said: “Following the surgery, on top of her dementia she had delirium. I think it was caused by the pain from the fracture, the pain killers and the element of dehydration and kidney injury.”

A post mortem revealed Mrs Moseley also suffered from bronchopneumonia.

Recording a conclusion of accidental death, Mr Wilkinson said: “There was a tragic downward spiral of events due to contributory factors and a chain of events which goes back to the fall at the home.

“She didn’t every fully recover from the fall. Marjorie is clearly a very missed mother and grandmother who was very fortunate to have a close family. Family played a huge part in her life.”

He said Mrs Moseley’s family did everything they could to help her and her husband live independently.
