Supporting people with chronic illness

A SELF-management course to help people who live with a chronic illness is being run by Hastings and Rother Primary Care Trust.

The Living Well '“ Expert Patients Programme has been developed for people who live with a long-term health condition, such as heart disease, back pain, arthritis, diabetes, MS or ME.

The course encourages people to use the skills and knowledge they already have to improve their quality of life.
